The error with _fsize_t appears to be due to an error with the way it is included.

From mingw/include/io.h lines 96-105 :

    /* We must define _fsize_t, but some compilers (including GCC prior to

I searched mingw/include for _fsize_t and it is not declared in wchar.h. So the problem may be including wchar.h before io.h which causes _fsize_t to remain undefined.
