[PATCH v9 59/61] dax: Convert to XArray

The DAX code (by its nature) is deeply interwoven with the radix tree
infrastructure, doing operations directly on the radix tree slots.
Convert the whole file to use XArray concepts; mostly passing around
xa_state instead of address_space, index or slot.
